Patient Information

First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, root canal treatment can be done the same day as the consultation.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring your insurance card.

This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

Financial Policy

Please understand that payment of your bill is considered part of your treatment. As the patient, you are responsible for the full payment regardless of insurance involvement. As a courtesy, we will file your insurance and collect an initial partial payment at the time of your appointment. Our office respects your privacy. The privacy of your health information is of utmost importance to us. We comply with any applicable state or federal privacy laws and maintain high standards of discretion and professionalism. In communicating with other dentists about your care, only required information is discussed. Information about procedures submitted to insurance companies only include the information needed to complete the submission of the claim on your behalf.
